$3,000 Green Bay Packers Foundation Grant Helps SSM Health Samaritan Clinic Address Patient Basic Needs

A $3,000 grant from the Green Bay Packers Foundation will support the SSM Health Samaritan Clinic and Agnesian HealthCare Foundation’s efforts to address social determinants of health by helping patients access basic necessities that impact their health and well-being.

Funds from the Green Bay Packers Foundation grant will be used to purchase coats, hygiene supplies, T-shirts, sweatshirts and sweatpants for patients in need. These items help address immediate needs identified during clinic visits and allow staff to provide practical support when patients are facing financial hardship.

“We are incredibly grateful to the Green Bay Packers Foundation for investing in the health and well-being of our neighbors,” said Deb Winterhack, Agnesian HealthCare Foundation director of development, Greater Fond du Lac. “This grant allows us to provide practical essentials that many people take for granted but can make a meaningful difference for individuals and families facing difficult circumstances. Community support like this helps ensure the SSM Health Samaritan Clinic can continue caring for some of the most vulnerable members of our community.”

The SSM Health Samaritan Clinic’s mission is to provide quality health care to uninsured and low-income patients while helping create a healthier community. In addition to medical services, the clinic connects patients with resources that address barriers to health, including food, clothing, hygiene items and other basic necessities that support their daily well-being.

“Many of our patients come to us with needs that extend beyond medical care,” said Missy Tate, SSM Health Samaritan Clinic leader. “Having access to basic necessities such as warm clothing and hygiene products can improve a person’s health, dignity, and quality of life. This generous gift from the Green Bay Packers Foundation will help us provide immediate support to patients when they need it most and continue our mission of caring for the whole person.”

The Green Bay Packers Foundation supports charitable organizations throughout Wisconsin that are working to improve the quality of life in their communities. Through this grant, the foundation is helping the SSM Health Samaritan Clinic expand its ability to meet essential patient needs and support healthier outcomes for individuals and families in Fond du Lac and Dodge counties.
